During a cosmetic surgery in December last year, a clinic in Solna, north of Stockholm, made a mistake about which trachea was which, reports Mitt i.
Instead of getting the air tube in the trachea, the woman got it in her esophagus during anesthesia. The woman was taken to Karolinska Hospital and received intensive care, but her life could not be saved.
After the death, the Health and Social Care Inspectorate (Ivo) initiated supervision against the clinic and in September, they directed sharp criticism for several serious deficiencies.
"It cannot be ruled out that it has affected the course of events that resulted in a patient's death," writes Ivo in its decision, according to Mitt i.
Among other things, the clinic had deficiencies in routines and patient safety work, including a lack of routines for acute situations, according to Ivo.
The clinic states in a response to Ivo that as a result of the death, they have decided to preliminarily discontinue the entire operation and cancelled all planned surgeries. They also report that they have rectified most of the deficiencies pointed out by Ivo.
The death is being investigated by the police as manslaughter and one person has been suspected.
